Sanctioning Forms
Read through the sanctioning guidelines before completing the forms.  To apply for sanctioning, the event organizer must complete the sanctioning application below and return it along with the deposit to the UPA.

Need help completing these forms? 
  • See information and tips for completing the Insurance Request Forms.
  • If you are providing your own event insurance, read details about UPA insurance requirements.
  • All electronic forms are word documents.  Simply open the document and complete it by tabbing through the form.  Save the completed document to your computer and email it to Sanction_event@upa.org as an attachment.
  • To download the manual forms, use a computer that is connected to a printer, the Internet, and has Adobe Acrobat.
  • Go to Team Registration for forms that teams and players can use to register for a sanctioned event.
